A 3-year-old boy in Arizona is dead after what authorities say was a "close call" with child protective services, the Arizona Republic reports.
Police say the boy was left alone in his family's home for hours while his parents went to work, and when they returned, they found him dead in a playpen.
According to the New York Daily News, authorities believe the boy had been left alone for at least an hour.
